Summary:

The Registered Nurse Transfer Center is responsible forevaluating, coordinating, processing, screening, and documentationof patient transfers into the CHRISTUS Health System. The CHRISTUSTransfer Center RN will collaborate with relevant business partnersfor appropriate level of care and transport of patients into theCHRISTUS Health system. The CHRISTUS Transfer Center will beresponsible for obtaining appropriate medical and demographicinformation and coordinating communication between the referringand receiving physician as well as between healthcare facilities.The CHRISTUS Transfer Center RN will efficiently engage appropriatedepartments in the transfer process. The CHRISTUS Transfer CenterRN will utilize problem solving and customer service skills todetermine the best course of action for the patient, the physician,and the hospital by working closely with facility HouseSupervisors, referring physicians, ED and inpatient staff to ensurethe effective and efficient transfer of every patient. Thisposition requires the full understanding and active participationin fulfilling the Mission of CHRISTUS Health. It is expected thatthe associate demonstrate behavior consistent with the Core Values.The associate shall support CHRISTUS Health's strategic plan andthe goals and direction of their Performance Improvement Plan(PIP).

Requirements:

• Associate's Degree inNursing
• RN License in state of employment orcompact
• BLS
