Provide comprehensive wound care and education to patients with chronic wounds, ensuring optimal healing outcomes and patient comfort.

Job Roles

Conduct basic wound assessments, including measurement and staging.
Good understanding of underlying factors impairing wound healing (eg chronic venous insufficiency, peripheral arterial disease, diabetic neuropathy, etc.) to facilitate patient education in their disease process and treatment.
Monitor wound progress and response to treatment, and report same to NP.
Educate patients, families, and caregivers on proper wound care techniques, prevention strategies, and signs of wound deterioration.
Collaborate with interdisciplinary healthcare team members, including physicians, physical therapists, and nutritionists, to optimize patient outcomes.
Adhere to infection control protocols and ensure a safe, clean environment for wound care procedures.
Stay informed about best practices in wound care management through continuing education and professional development.
Participate in quality improvement initiatives aimed at enhancing wound care services and patient satisfaction.
Performs a wide variety of patient care activities and accommodative services for assigned Wound clinic patients, as directed by the Wound Clinician (NP, PA, DO, MD) and/or Director of Nursing, to facilitate the delivery of prompt, quality care.
Completes all required documentation in the electronic health record accurately, in a timely manner, and thoroughly in accordance with department standards.
Performs other duties as assigned.

Requirements:

Current Licensed Vocational Nurse (LVN) or Registered Nurse (RN), licensed in the state of California.
Bachelor's degree in nursing (BSN) preferred.
Certification in wound care nursing (WCC, CWOCN, or equivalent) preferred, but not required.
Minimum of two years of clinical experience in wound care nursing is required

If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors - we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
