At Summus Health Care, we seek to expand our Chronic Care Management team by adding a Licensed Vocational Nurse with primary care experience who can assist us in providing outstanding care.An active Texas LVN license with no discipline is required for this role.

Previous experience in primary care is required.What will you do?Utilize critical thinking abilities and problem-solving strategies in the management of chronic careMaintain current knowledge of disease processes, resources, and treatment options available.Achieve the set performance targets.

HIPAA-compliant use of computer access to facilitate patient care.Use a care plan to guide patient interaction and direction.Review individual care plans and make monthly phone calls to CCM-enrolled patients.

Manage the flow of information to and from the providers office.Assists in the assessment of patient needs and health by taking patient history, medication listing, and assessing patients physical and mental status.Complete all necessary documentation in the patients medical record in accordance with the practices charting policy.

Support Annual Wellness Visits by contacting patients to schedule appointments and collect data.All other duties as assigned by manager.
