Job Requirements:

• FL RN (Registered Nurse) license
• Education: Associates Degree in Nursing
• Experience: Minimum of three years of Acute Care RN experience within a hospital setting required.
• Minimum 2 Years of RN Case Management within a Large Acute Care "Hospital".
• 1 year of Utilization Review experience required

About the Job:

• Working closely with the collaborative care team, the case manager assesses plans, intervenes, and reassesses an individual's needs to implement a plan of care to meet the needs of that individual
• The case manager assesses patient and family for clinical appropriateness for the setting and establishes plans for effective allocation of resources and management of the continuum of care.
• Utilization review is conducted according to ShorePoint Health guidelines
• Coordinates the overall interdisciplinary plan of care for a patient, from admission to discharge
• Acts as a liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively.
• Day Shift Available (8X5's varying days)

• Minimum five years of clinical e experience in an acute care setting.

• Previous case management and/or utilization management/utilization review experience.

• Associates or better in Nursing
• High School or better

• Bachelors or better in Nursing or related field

• L-FL-RN License

• C-AHA BLS Basic Cardiac
