Nurse-led management of chronic conditions
- Preventative healthcare, immunisations and cervical cytology
- Health promotion and supporting lifestyle change

Essential requirements
- Registered Nurse with 2+ years post-registration experience
- Minimum 1 year General Practice experience
- Recent community nursing experience
- Competence in immunisations, cervical cytology, ear care and minor surgery
- Experience managing chronic conditions and working to protocols
- Strong communication, leadership, IT and audit skills
